Blood Education Center, LLC (BEC), 2850 Willow Sprout Road, Lehi, Utah 84043, offers to BEC client’s access to its website (and related data) exclusively for the purpose of providing timely services to improve client care. This agreement includes, but is not limited to, secured access to client's patient information, including test orders and result data. The period of this agreement begins upon placement of an order. So long as the service is utilized in accordance with BEC specifications and the terms of this Agreement, BEC will not charge the client a monthly fee or cancel access.

BEC agrees to the following:

Provide accurate and timely test results for the data retention period of a minimum of 365 days from test completion.

Client agrees to the following:

Provide Internet access to received results by email. Minimum browser requirements are Version 4 (four) of either Netscape or Internet Explorer. There are no communication speed requirements.

All patient results viewed through Internet Inquiry are not considered the "official" printed patient report. BEC will continue to provide, through current communication means to the client directly, all patient reports.

In the course of using BEC Blood Work Services, client will have access to written or electronic media information concerning: test orders, test results, and services performed by BEC. Client will not disclose (verbally, in written form, by electronic or any other means) to any unauthorized person, individual patient health information to which they have access through BEC Blood Work Services. Client agrees to comply with security and privacy regulations to which the client is subject.

The general intent of this access is for use by BEC clients for "testing" information provided by BEC. It is the client's sole responsibility to determine, control and be responsible for those people who are allowed access.

Client will use the access solely for purposes granted. Client agrees to not intentionally disclose access to their email account where blood work results may be located.

Authorization/Documentation

This agreement, with its detailed and intended cooperation and responsibilities, is accepted by virtue of checking the box labeled "I have read and agreed to your privacy statement," and remains in effect as long as the client has authorized access to BEC Blood Work Services.
